You do not require a doctors sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website
If you need a first-time fit note, please make an appointment with a GP.
To request a repeat fit note or an extension of an existing one, please follow the above link to submit a PATCHs request.
If you are unable to use the PATCHS online consultation tool to request a repeat sicknote then please contact the surgery via phone.
